PURPOSE OF THIS BINDING AGREEMENT
By reading and signing this document, “You,” sometimes also referred to as “my” or “I,” will agree to release and hold Kelly Sisley (also referred to as “trainer”), harmless from, and assume all responsibility for, all claims, demands, injuries, damages, actions or causes of action to persons or property, arising out of or connected with my use of training/coaching services with Sisley Health, LLC, The Biohacking Transformation Program (BTP) & apps/websites/deliverables used. NSCA-CPT trainer Kelly Sisley will make various efforts to minimize any potential risks in all areas.
I am aware that exercise has some potential side effects and risks. It is possible throughout exercise, training sessions or my program that I may experience abnormal blood pressure, irregular heart rhythm, dehydration, fainting and/or dizziness. It is also possible that I might seriously injure myself from the use of exercise equipment, failure of exercise equipment, tripping or falling, or other hazards associated with equipment, moving around while exercising, and my surroundings. In very rare circumstances, it is possible that exercise can cause heart attack, stroke or death. It is extremely important that any physical or other symptoms that I experience whilst participating in the program are explained to my Doctor and/or Trainer, if applicable, even if I feel that they might not be important. It is also important that I tell my Doctor and/or Trainer, if applicable, any information I possess about my health status, or changes to my health during the course of my program, especially those that relate to heart problems including shortness of breath, Informed Consent and Release of Information for Participation in Exercise pain, pressure, tightness or heaviness in the chest, neck, back, jaw, calf area and/or arms. By telling my Docotor and/or trainer this information I am minimizing my risk or injury, complications and death. It is expected that I will tell my trainer all medications I use, begin to use or cease using (including non-prescription) prior to participation in my initial or regular training sessions if participating in 1:1 training. It is also expected that any short term changes to my usual medication regime are reported to my trainer (e.g. forgetting to take my medication one morning).

FASTING
This program will at times include fasting protocols that you may choose to or not to participate in. Fasting protocols are not recommended for the following:
People with eating disorders or are frail/underweight, Children under 18, Type-1 diabetics and diabetics taking diabetes medication (other than Metformin), pregnant or breast feeding, people recovering from surgery, those who have a significant underlying medical condition or are not sure about whether it may affect their prescribed medications should to speak to their doctor first, people not feeling good, sick, or have a fever, people taking Warfarin should consult their Doctor first - it may increase INR, or people with a history of serious mental health problems.
I understand all of the information and instructions outlined in this informed consent, have had time to discuss any concerns with a health professional, and considering this, agree to participate in this coaching program at my own risk. I also agree to release and indemnify Kelly Sisley, Sisley Health, LLC and the BTP from or against any actions or claims arising from any injury, loss, damage or death caused to me. I also give permission for Kelly Sisley to use my image / name / progress anonymously in any promotional social outlets/websites online. I hereby consent to voluntarily engage in the exercise program considering the above information. I understand what is expected of me and the risks and procedures associated with this program. After fully reading this document I voluntarily consent to participate in the exercise program.
YOU HAVE READ THE FOREGOING, ACKNOWLEDGE THAT YOU UNDERSTAND THE TERMS AND CONDITIONS SET FORTH IN THE PRECEDING PARAGRAPHS, AND AGREE TO SAME.
